The Board updates its records to process a name change that has been submitted by a licensee if the name change is the legal name for all purposes. Name changes must be submitted in writing with the appropriate court documents accompanying the request.
A licensee may request a new wall certificate and license renewal cards to reflect the name change by checking the appropriate box on the form. Please see form for further instructions.

A licensee shall notify the Board within thirty (30) days of any change of his or her legal name, primary address, telephone number, email address or similar change of location or status. Please print and complete the'Update Form' provided.
Replacement License Certificate: Please use the 'Update Form' to request replacement documents for the wall certificate or license renewal cards. (Please see form for more information.)
