The Board updates its records to process a name change that has been submitted by a licensee if the name change is the legal name for all purposes. Name changes must be submitted in writing with the appropriate court documents accompanying the request.
A licensee may apply for a license replacement document to reflect the name change by submitting a written request with the appropriate documents accompanying the request and the appropriate fee.
A licensee shall notify the Board within thirty (30) days of any change of his or her legal name, primary address, telephone number, email address or similar change of location or status. Please print and complete the 'UPDATE FORM' provided. Please mail to the board address below:
