If your LPC license number is 2084 or below, (issue date of 06/30/2013 or below) you have the following two options to renew your license:
(Per Series 3, LPC Renewal Rules, this LPC will not submit the CE certificates of completion at this time. A random audit will be conducted at a later date and those licensees randomly selected will be notified via email.)
The LPC will have 20 minutes to complete the online application. Please be sure you have your information ready and available in order to enter your information within this timeframe. If you exceed 20 minutes, the link times out and all information entered is lost and you will be required to start over in order to complete the online method.
Please be sure to print each completed page (ctrl + P) before moving on to the next so that you have a printed copy of your submission. This is the only method of obtaining a copy of your completed online renewal.
The final screen will be the credit card payment page in which you will make your payment online. Be sure to enter your email address when prompted.
After your submission and credit card payment, you will receive an email of payment confirmation from the WV Treasury Dept for your credit card payment. This does not include a copy of your entries for the online application. Please be sure you follow the tip above and print your entries as you navigate to each page within the online application. If you haven't received the email within 30 minutes, check your 'junk' folder as sometimes the email lands here.
If after following the instructions provided below for downloading the PDF document, you are still unable to successfully download the document, please click here to download and PRINT the Renewal Packet in WORD format.
In order to access the PDF document, you will need the Adobe Reader program (free) found at the following link: https://get.adobe.com/reader/. PLEASE NOTE: After downloading the program, you will need to save the Renewal PDF document to your desktop first, then open the PDF document from your desktop and the document will be ready for view and completion.
After downloading, be sure to double click the forms in order to enlarge. Please first print the instruction sheet and the checklist before proceeding and use as your guide as you complete the your documents.
When you reach the two (2) page LPC Renewal Application and the LPC CE Recording forms, double click once again to begin entering your data.
After completion, the LPC will print the documents, sign and date the 2 page application and mail the materials to the Board address provided on the form.
If you do not want to use the PDF form as an auto fill document, you may print and write in your responses. Please print legibly to ensure we include your correct information in the LPC Database.
